client session log: 5 copies needed
Student must complete 50 client treatment sessions before registering for the certification program. At least half of the sessions must be 100% of an hour of Craniosacral therapy. The other half may be 50% of an hour session of Craniosacral therapy, with the other 50% being any other modality of the student’s choice.
Per HIPPA, avoid using full names on the form. Just use their initials or other identifier.
Client treatment sessions should be documented on the following Client Session Log. Student will need five copies of the form for 100 sessions. Student will hold on to all five copies until all 100 sessions are completed, then Instructor will sign the tri-fold pamphlet.
SOAP charts are NOT required for the program, but may be required per student’s individual office policy.
